I hereby authorize Dynamic Staffing Group and any third-party consumer reporting agencies or background screening providers engaged by Dynamic Staffing Group, to conduct a criminal background check.
This authorization includes, but is not limited to, inquiries into my criminal history records at the federal, state, county, and local levels, including records maintained by law enforcement agencies, courts, and other governmental or private entities.
I understand that this background check may be conducted for employment, contract, placement, or continued engagement purposes.
Disclosure and Consent
I acknowledge that:
The information obtained through this background check may be used by Dynamic Staffing Group in making decisions regarding my eligibility for employment, assignment, or continued engagement.
I may be required to provide additional information or documentation to verify my identity.
I have the right to request a copy of any consumer report obtained, if applicable, in accordance with the Fair Credit Reporting Act (FCRA) and any applicable state laws.
Waiver and Release of Liability
I hereby release, discharge, and hold harmless Dynamic Staffing Group, its officers, employees, agents, and representatives, as well as any background screening agencies or information providers, from any and all claims, liabilities, damages, or causes of action arising out of or related to the lawful collection, disclosure, or use of information obtained during the background check process.
I certify that all information I have provided is true, complete, and accurate to the best of my knowledge. I understand that providing false or misleading information may result in disqualification from employment or termination if already engaged.
I acknowledge that I have read and understand this authorization and voluntarily consent to the criminal background check described above.
